Tank treads fall off during turns

I built a bot with tank treads, this one here actually: http://www.nxtprograms.com/NXT2/multi-bot/index.html, and I've been having issues driving on carpet. For turns in place one or both of the treads seem to eventually come off after a few turns.

Any design advice on how to get the treads to stay on? I was thinking of separating the wheels by another notch so that the treads are on "tighter", as they seem a bit loose, but I'm not sure if that is possible or the best idea.

Any suggestions are greatly appreciated.

You could extend the wheel axle's and add a bar on the outer side of the wheel, mounted on the extended axle. To further secure this bar you can place axle's and bars through the middle of the track section.

Another approach would be to add another wheel to the track, however this would be most effective used in combination with the approach above.
